Please fill out and submit the following Patient-Teacher Request Form if you are an educator. To the right side of this page is information on what you may expect once you make your request.

Once you submit a request:

VMP Genetics will contact you within 1 week if there are questions about your request.
VMP Genetics will contact up to two Patient-Teachers with the requested diagnosis and notify him/her/them about the request.
VMP Genetics will contact you within 1 - 3 weeks with a response to your request.
If the Patient-Teacher is agreeable to the request, he/she/they will contact you directly or VMP Genetics will make the introduction.
VMP Genetics will follow up after the engagement to ask about the event.

Please note, only one Patient-Teacher is contacted at a time. Only if the first request is not available is the second Patient-Teacher contacted.
